Non-Surgical removal of residual biliary stones by postoperative choledochofiberscope


Abstract
The management of residual stones in hepatobiliary system has been largely dependent on reoperation, until the introduction of several methods of management that were recently developed.
Papillotomy through endoscope, and the mechanical extraction through X-ray guidance have greatly reduced the frequency of reoperation even with the aid of the chemical dissolution of stones.
But each procedure and method had its own limitation.
From March-1982 till October-1983, 43 choledochoscopy procedures, conducted by insertion of : choledochofiberscope into the biliary tract through the sinus tract after T-tube had been removed, were carried out in 24 patients at Korea University, Kuro Hospital.
In this series, 8 with retained biliary tract stones were encountered, and complete removal of stones was successfully carried out in 7, using postoperative choledochofiberscopy.
The following study in cases of complete extraction of the retained biliary tract stones showed that this approach was most effective and safe. Furthermore, I advise routine use of postoperative choledochofiberscopy as an adjunt to the T-tube cholangiography in cases of inconclusive cholangiographic findings, in order to prevent the possibility of retained bilily tract stones.
